Obesity: Understanding It as a Chronic Disease


For many years, obesity was often seen as simply the result of overeating or lack of exercise. But modern science tells a different story. Today, major medical organizations—including the World Health Organization (WHO), the American Medical Association (AMA), and the Centers for Disease Control and Prevention (CDC)—recognize obesity as a chronic disease.

This shift changes how we understand obesity and, more importantly, how we treat it.

What Does It Mean to Call Obesity a Chronic Disease?

A chronic disease is a long-lasting condition that often requires ongoing medical care and lifestyle management. Obesity fits this definition because:

• It is persistent: Weight gain and its health effects typically last over time.

• It involves complex biology: Genetics, hormones, environment, and behavior all play roles.

• It has serious complications: Obesity increases the risk for type 2 diabetes, heart disease, certain cancers, and more.

• It needs long-term care: Like hypertension or asthma, it often requires lifelong management, not just short-term fixes.

The Biology Behind Obesity

Obesity is not just about willpower—it’s influenced by multiple systems in the body.

• Genetics: Research has shown that approximately 40 to 70% of body weight differences may be linked to inherited traits.

• Hormones: Body fat produces hormones that tell the brain whether energy stores are low or high. If fat stores are low, the brain signals the stomach to produce more or less of a hunger hormone called ghrelin. If fat stores are high (with caveat below), then an anti-hunger hormone called leptin tells the brain that it does not require food.

* With weight loss: leptin from body fat decreases and ghrelin increases, so food intake will increase. Also, people who have lost weight now burn fewer calories at rest. For both of these reasons, it makes it harder to maintain weight loss over time.

* With weight gain: over time, high levels of body fat means that there can be too much leptin production. With this, the brain simply may become less responsive to leptin’s anti-hunger effects. This causes more hunger to occur when, in fact, shouldn’t be the case. This is called leptin resistance.

• Brain pathways: The brain’s reward system can drive cravings for high-calorie foods, making weight regulation more challenging.

• Environment: Easy access to processed foods, sedentary jobs, and lack of safe spaces for physical activity all contribute.

This complexity is why quick fixes—like crash diets—rarely work in the long term.

Why Recognizing Obesity as a Disease Matters

Labeling obesity as a chronic disease is not about “medicalizing” body weight—it’s about improving care and reducing stigma.

1. It changes treatment approaches

Healthcare providers can address obesity with evidence-based strategies, including medical nutrition therapy, behavioral therapy, medications, and in some cases, bariatric surgery.

2. It reduces stigma

For decades, people with obesity faced judgment and shame. Recognizing obesity as a disease shifts the focus from personal failure to a medical condition that deserves compassion and treatment.

3. It supports prevention

Framing obesity as a disease also highlights the importance of early prevention—through healthy environments, supportive policies, and education. It’s much easier to prevent obesity when we are young than it is to treat as we become older.

Treating Obesity Like Other Chronic Diseases

Just as high blood pressure isn’t “cured” by a week of medication, obesity management requires ongoing care. Effective approaches often include:

• Lifestyle changes: Balanced eating, physical activity, and stress management remain the foundation.

• Medical therapies: FDA-approved medications can support weight loss by regulating appetite and metabolism.

• Surgical options: Bariatric surgery, when appropriate, has been shown to improve weight and reduce related health risks.

• Continuous support: Counseling, group programs, or coaching help people sustain long-term changes.

Obesity is more than a matter of calories in and out—it’s a chronic disease influenced by biology, behavior, and environment. Recognizing it as such is a critical step toward providing better care, reducing stigma, and improving long-term health outcomes.

Instead of viewing weight management as a short-term goal, thinking of it as an ongoing journey that primarily focuses on healthy living. This approach offers a more sustainable and realistic path forward.
